Medicaid PatientsGBUACO, New York’s first Medicaid Accountable Care Organization, is a group of physicians, hospitals, and other health care providers who come together voluntarily to give coordinated high quality care to the Medicaid population in Western New York. Coordinated care helps ensure that patients get the right care at the right time, and seeks to avoid unnecessary duplication of services and preventing medical error.

GBUACO providers benefit from the following:

  1. GBUACO’s provider-led network, with a strong base of primary care practices collectively accountable for quality and total per capita costs across a continuum of care for a patient population;
  2. Provider payments linked to quality measures and quality improvements designed to reduce overall costs;
  3. Reliable performance measurement to support improvement and meet metrics for shared savings.

Physician practices benefit from partnership with GBUACO by participating in innovative financial incentives included when delivering care to patients in the ACO model. GBUACO has its’ own Patient Centered Medical Home Transformation Team primed to facilitate transformation of a practice to meet metrics necessary for ACO participation as a Patient-Centered Medical Home.

Within the GBUACO network, the healthcare focus shifts to being patient-centered. Utilizing telemedicine and an effective electronic health record aids in reduction of administrative processes that usually contribute to redundancies and unnecessary care, while improving access to healthcare information for both the patient and the doctor. Providers within GBUACO are poised to deliver quality, coordinated care that results in reduced costs and an improved patient experience.
