Dr. Raul Vazquez, M.D.F.A.A.F.P.Dr. Raul Vazquez, MD, FAAFP
Urban Family Practice
President/CEO

Dr. Raul Vazquez is a primary care physician who opened Urban Family Practice on Buffalo’s Lower West Side in 1996. In 2016, the National Committee for Quality Assurance (NCQA) announced that Urban Family Practice of Buffalo had received level three NCQA Patient-Centered Medical Home (PCMH). Level three is the highest degree of recognition available.

Recognizing the need for coordinated, person-centered care for Medicaid recipients with multiple chronic medical conditions, he formed the Greater Buffalo United Accountable Healthcare Network (GBUAHN). The New York State Department of Health approved the designation of GBUAHN as a provider-lead Medicaid health home in 2012.

As president of GBUAHN, Dr. Vazquez leads a team of dedicated professionals in the medical and business fields. He has set the strategy and direction for the fast-track growth of GBUAHN. Under his guidance, GBUAHN has grown to serve several thousand members and employs a staff of approximately 150 individuals. GBUAHN is one of the largest minority employers in Western New York.

In June 2009, Dr. Vazquez was nominated by Governor David Paterson, and approved by the New York State Senate, to serve as a member of the New York State Medicaid Advisory Committee. In June 2012, Governor Andrew Cuomo and the New York S Senate approved Dr. Vazquez to serve on the Minority Health Council.

He has won more than 20 awards for his accomplishments in business, the medical field and advocacy groups. Dr. Vazquez has served boards for numerous organizations, including: Upstate New York Transplant Services, HEALTHeLINK and WNED-TV.

Dr. Vazquez received his undergraduate degree from Fordham University and received his medical degree from the State University of New York at Buffalo.

Dr. Kenneth Gayles, M.D.Dr. Kenneth L. Gayles, MD FACC
Gayles Medical
Vice President

Dr. Kenneth Lee Gayles is a cardiovascular specialist who treats over 6,000 cardiac care patients annually. He is the owner of Gayles Medical practice. Dr. Gayles is board certified in cardiovascular diseases and internal medicine and brings to GBUAHN over four decades of experience diagnosing and treating congenital heart defects, heart failure, coronary artery disease, and other heart problems. He was the first African-American invasive cardiologist in Western New York.

As senior vice president of GBUAHN, Dr. Gayles’ mission is to deliver a comprehensive health care system targeting the underserved community in Western New York. His goal for GBUAHN is the elimination of disparity in health care delivery and outcomes in the Western New York community.

Dr. Gayles served as chairman of the board of Sheehan Memorial Hospital from 1990 -1991 and served as medical director until 1995. He earned his Bachelor of Science degree in biology and his medical degree from the State University of New York at Buffalo. He completed his internal medicine residency and cardiology fellowship at SUNY Buffalo.

Dr. Gayles has served as treasurer of the National Association of Black Cardiologists, and as vice president of the National Medical Association - Buffalo Chapter. His other community affiliations include the Buffalo Philharmonic Board of Directors and the Buffalo PREP Board of Directors. He is a trustee for the First Shiloh Baptist Church and has served as chief of medicine at Deaconess Hospital. He is also member of the Board of Trustees of the UB Foundation.


Dr. Frances Ilozue, M.D.Dr. Frances Ilozue, MD

Dr. Frances Ilozue received her board certification in 2007. That same year she opened a private practice - Rapha Family Medicine, PC - located at 2200 Main St. in Buffalo, N.Y. The practice provides comprehensive primary care services with emphasis on disease prevention and optimal chronic diseases management through patient-doctor partnership.

Dr. Ilozue is an active member at Bethesda World Harvest International Church. She also works with the International Mission of the Rapha Health and Wellness Foundation, focusing on disease prevention and chronic disease management in medically underserved regions. Dr. Ilozue is active in other faith-based and health-related community outreach programs including Career Mentorship for Adolescents and Young Adults Interested in Health Care.

Dr. Ilozue completed her medical education at the State Medical Academy, St. Petersburg, Russia. Prior to settling in Buffalo, N.Y., Dr. Ilozue held positions in Enugu and Lagos in Nigeria. She completed her residency at the State University of New York at Buffalo Internal Medicine/Pediatric Residency Program and at Conemaugh Memorial Medical Center in Johnstown, Pennsylvania.


Dr. Riffat Sadiq, M.D.Dr. Riffat Sadiq, MD
Treasurer

Dr. Sadiq earned her medical degree from Fatima Jinnah Medical University in Lahore, India. She also completed a residency at State University of New York at Buffalo Graduate School of Dental Medicine, and a fellowship through the State University of New York Consortium Geriatrics Fellowship Program. She is currently president and chief executive officer of WNY Medical, PC, an independent physician association which she founded in 2002. This multi-clinic and multidisciplinary practice focuses on primary care and disease management and prevention. Dr. Sadiq leads a team of 35 physician and midlevel providers serving in eight offices throughout Western New York, as well as providing inpatient services at several area hospitals and nursing homes.

Dr. Sadiq is also the president of Geriatric Associates, LLP, serving as the medical director of acute geriatric service at Erie County Medical Center. In addition, Dr. Sadiq is affiliated as volunteer faculty with the State University of New York at Buffalo and is a preceptor with the physician assistant program at Daemen College. Dr. Sadiq has been recognized by Business First of Buffalo as one of the top medical professionals in the Buffalo area for her contributions to improving health care in Western New York. She has been consulted by TV and print media to render her opinion on health and healthcare issues.

Dr. Kenton Forte, M.D.Dr. Kenton Forte, MD
Buffalo, NY

Dr. Kenton Forte has been practicing internal medicine at the Heart Beat Center of WNY for well over 20 years. He also specializes in cardiovascular disease. His hospital affiliations are Buffalo General Hospital and ECMC.

In addition to practicing medicine at the Heartbeat Center of WNY, Dr. Forte has been an assistant clinical professor at the State University of New York at Buffalo for more than two decades.

Dr. Forte is a member of eleven professional societies, including the American Heart Association, American Society of Internal Medicine, and the Erie County Medical Society. He has written four publications for various academic journal and presented at numerous symposiums and conventions on his expertise in cardiology.

Dr. Forte completed his residency and fellowship in internal medicine and cardiology at the State University of New York at Buffalo.


Dr. Jesslyn Perry, M.D.Dr. Jesslyn Perry, MD
Buffalo, NY

Dr. Jesslyn Perry has been a physician since 1995. After working at Sheehan Hospital as an internist from 1999 -2001, she opened Ella Perry, M.D., located at 191 North Street in Buffalo, N.Y. She named her practice after her mother.

Dr. Perry specializes in family and internal medicine. She is affiliated with Women and Children’s Hospital of Buffalo and Buffalo General Hospital. She is board certified in Internal medicine. Dr. Perry is a member of the National Medical Association (NMA).

Dr. Perry is a graduate of the State University of New York at Buffalo School of Medicine and Biomedical Sciences.


Dr. George Haddad, M.D.Dr. George Haddad, MD
Tonawanda, NY

Dr. George Haddad is N.Y. State licensed and board certified in internal medicine. He has been in private practice since 1994. He is affiliated with Kenmore Mercy Hospital and Catholic Medical Partners.

Dr. Haddad previously had affiliations with Sheehan Hospital and Coastal Emergency Room Services.

Dr. Haddad attended the Catholic University of Louvain, Belgium, where he graduated with honorable distinction.


Dr. Winston Douglas, M.D.Dr. Winston Douglas, MD
Douglas Primary Care

Dr. Winston Douglas is certified in internal medicine by the American Board of Internal Medicine (ABIM). Dr. Douglas is well-versed in the health problems of aging populations with Alzheimer's disease, Parkinson's disease, heart conditions and pulmonary diseases.

As treasurer of GBUAHN, Dr. Douglas develops strategies regarding short and long term financial vitality.

Dr. Douglas served as medical director of Waterfront Nursing Home. He is the former chair of the Pharmacy and Therapeutic Committee at Sheehan Health Services, and the former medical director of N.Y. State’s Orleans Correctional facility. Dr. Douglas is a member of the National Medical Association - Buffalo Chapter and he is the current medical director at Alcohol and Drug Dependency Services.

Dr. Douglas received his medical degree from State University of New York at Buffalo School of Medicine.


Dr. Dwight Lewis, M.D.Dr. Dwight Lewis, MD
Main Primary Care

Dr. Dwight Lewis is an internal medicine specialist with a subspecialty in sepsis blood poisoning. He opened his medical practice, Main Primary Care, in 1994 in Buffalo’s University District. Dr. Lewis is an advocate of screenings, wellness check-ups, vaccinations and healthy living in order to prevent disease and help his patients stay well.

As secretary of GBUAHN, Dr. Lewis manages the records and administrative functions of the GBUAHN Board of Directors.

Dr. Lewis graduated in 1991 from the State University of New York at Buffalo School of Medicine and Biomedical Sciences. He is admitted to Sisters of Charity Hospital and Kaleida Health - Buffalo General.

Dr. Lewis is a member of the National Medical Association (NMA).


Dr. W. Sam Yi, M.D.Dr. W. Sam Yi, MD

W. Sam Yi, MD is CEO of CCS Healthcare, which is a multi-disciplinary medical practice that provides comprehensive clinical services in Western New York. Dr. Yi joined CCS Oncology as staff physician in 2006 and became the medical director and president in 2008. Under his leadership, CCS has grown tremendously in terms of geographical coverage and various medical services available for patients. The vision of CCS is to provide a highly integrated medicine in a community-based model (not hospital-based).

Dr. Yi is a board-certified Radiation Oncologist (the American Board of Radiology) specializing in malignancies of breast, prostate, CNS, and lung. He completed his undergraduate studies at Columbia University in environmental engineering and medical education at the University of Rochester School of Medicine. For his residency, he stayed in Rochester and completed his training in radiation oncology at the James Wilmot Cancer Center/ University of Rochester Medical Center. He has been recognized for his research (awards and presentations) by various organizations (Radiological Society of North America, Radiation Therapy Oncology Group, American Society of Therapeutic Radiation Oncology, Upstate NY Society of Radiation Oncology). In addition, he has published numerous articles in peer-reviewed medical journals.


Dr. Mark Pundt, M.D.Dr. Mark Pundt, MD

After graduating from the State University of New York at Buffalo School of Medicine as a Doctor of Medicine, Dr. Mark Pundt became the Chief Resident in Emergency Medicine at Mt. Sinai Medical Center in Cleveland, Ohio. Pundt successfully completed the Emergency Medicine Board Certification Exam of the American Board of Emergency Physicians, then achieved the position of Chief Executive Officer for Professional Emergency Services, PLLC. Pundt also serves as the Chief Executive Officer of the Medical Division of Innovative Health Services of America, Inc. He continues to work clinically as an attending physician at Kaleida Health Systems’ Millard Fillmore Gates and Suburban Hospitals in the Emergency Department. Culminating years of research and planning including attending numerous conferences related to healthcare management led Dr. Pundt, along with Professional Emergency Services, PLLC to open MedFirst Urgent Care, PLLC on September 1st, 2004. His aim was to ensure a relaxed setting staffed by qualified medical personnel and physicians who would offer after-hours medical care emphasizing trust, respect, confidentiality and compassion. The success of MedFirst as a pioneer in the Urgent Care arena in the Western New York area attests to the diligence Dr. Pundt provides in his management capacity. MedFirst was rebranded as MASH Urgent Care in August of 2012. MASH Urgent Care has four locations in the Western New York area with three more locations opening within the next 6 months.


Dr. Joseph Serghany, M.D.Dr. Joseph Serghany, MD

Joseph Serghany attended the American University of Beirut Medical School in 1988 and graduated with an MD degree. He then completed his Radiology residency at the University at Buffalo in 1995. Dr.Serghany joined the universitys’ radiology practice at ECMC until the year 2000, where he was an assistant program director for the Radiology Residency program. Currently he is the Co-founder and President of WNY imaging group, with offices in Lockport, Kenton, and Buffalo, NY.

Faizan HaqFaizan Haq

Faizan Haq has been teaching Islamic Cultural History and U.S. and the Muslim World since 1996 at the State University of New York at Buffalo. He has been a lecturer for intercultural communication at Buffalo State College since 2004. In addition to Islamic studies, his academic interests include: intercultural studies, international relations, and media analysis. He also has taught courses on Islam and democracy and U.S. in the world. Presently, he is developing courses on Muslims image in the media and contemporary Middle East and its relations with the U.S. Faizan Haq is a known media analyst and has been asked to render his opinions for various media networks on diverse international issues including ABC, CBS, NBC, Voice of America, and YNN. He is also the founder of WNYMuslims.org.

He is a fellow of Association of Diplomatic Studies and Training (ADST), Washington, DC, a lifetime member of Asian Studies Association. Currently he serves on the Executive Board of New York Conference on Asian Studies (NYCAS). He is one of the founding members of the Center for the Study of Islam and Democracy (CSID), the co-founder and president of Pakistani American Research and Information Center (PARIC), and he is a fellow of American Institute of International Studies (AIIS) based out of Fremont, California. He has served as secretary and vice president of Association of Muslim Social Scientists (AMSS) and as secretary general and vice president of Pakistani American Congress (PAC). He has also served as the president of the Pakistani American Association of Western New York (PAAWNY).

Carlton BrockCarlton Brock
Chief Operating Officer (COO)

As chief operating officer, Carlton Brock is responsible for designing and implementing GBUAHN’s business strategies, plans and procedures. He oversees operations and facilitates the establishment of policies that promote the company’s culture and vision.

Previously, Brock served as senior vice president of sales at St. Jude Medical for the Northeast, United States division. During his tenure, Brock managed a staff of 260 individuals and helped St. Jude Medical gross $250 million in annual sales. Brock also worked as the regional vice president for Medtronic USA, Inc.

Brock received his Bachelor of Science degree from California University of Pennsylvania. He also earned a general management certification from Columbia University. Brock has been with GBUAHN since July 2016.


Luis A. SantiagoLuis A. Santiago
Chief Financial Officer (CFO)

Luis “Lou” Santiago has been involved with GBUAHN since its inception in 2012. He currently serves as chief financial officer. Santiago has been charged overall with a multi-faceted involvement in the start-up and maintenance of GBUAHN, including: substantial workforce expansion, and payroll and benefit management. He reviews contracts for every aspect of the corporation, including memorandums of understanding with downstream providers.

Santiago came to GBUAHN with diverse experience in the insurance, health care and not-for-profit sectors. He is a prudent manager, working toward creating efficient operational processes since GBUAHN’S inception. He has led interactive team dialogues on workforce engagement and ethical practices and prepared initial on-boarding and training for new hires in advance of a dedicated training team.

Santiago has a Bachelor of Arts degree from Daemen College and a Master of Business Administration degree from the State University of New York at Buffalo. He has achieved Green Belt Six Sigma, and continues his work toward a Certified Six Sigma Black Belt Designation with vertical in health care.


Toni VazquezToni Vazquez
Chief Systems Officer (CSO)

Toni Vazquez is the chief systems officer (CSO) of GBUAHN. She is also the co-owner and practice administrator of Urban Family Practice (UFP). UFP is a state-of-the art’ medical facility located on the West Side of Buffalo that serves over 14,000 patients annually.

Vazquez is a philanthropist, and has served on various committees and boards. She co-founded The Raul and Toni Vazquez Foundation. She is an elected member, and current president, of the Williamsville Central School District Board of Education. She is a lifetime member of The National Black MBA Association. Vazquez is a member of the Higher Heights’ Founders’ Circle. The Higher Heights Organization works to build, expand and sustain a national political infrastructure to strengthen black women’s political power and leadership capacity. She also serves on the Buffalo Zoo Board of Directors, Amherst Diversity Commission, Visit Buffalo Niagara Board of Directors, and the Buffalo/Niagara Convention Center Board of Directors.

Vazquez earned her Six Sigma certification from Villanova University. She is a graduate of the Women’s Campaign School at Yale University. She earned a Master’s of Business Administration from Medaille College, and a Bachelor of Arts degree in psychology from the State University of New York at Buffalo.


Ralph HernandezRalph Hernandez, MS/HSA
Executive Director

Ralph is a goal-driven, compassionate, and accountable professional with over 25 years of experience in public and private healthcare settings. Ralph received his advanced certification in Health Services Administration at D’Youville College and studied social sciences at Medaille College. Ralph served as special assistant to Erie County Health Commissioner, Dr. Arnold Lubin. He also served as Board Member for the Buffalo Board of Education, with two consecutive terms as President, and one term as Vice-President of Executive Affairs over the course of 9 years. He currently is an executive at New York States first Medicaid and Commercial ACO, GBUACO.


Jason IsbrandtJason Isbrandt
Director of Provider Relations

As the account executive/provider relations director for GBUAHN, Isbrandt serves as the liaison between GBUAHN and our partnering affiliates. He contributes to the continued growth of our provider base. Isbrandt fosters current provider relationships and is responsible for expanding and improving GBUAHN as a recognizable brand.

Isbrandt has 18 years of outside sales experience in sports team sales, advertising and pharmaceuticals.

Isbrandt earned his Bachelor of Science degree in sports management from Medaille College.


Carissa SanchezCarissa Sanchez
Marketing Director

Sanchez began her career at GBUAHN as a member services representative. In that role, she recognized the great potential, and the greater need, for marketing and media outreach. In a few months, she doubled followers on all GBUAHN social media websites and actively advocated for more organized marketing for GBUAHN. She established a distinct marketing department and team and is working diligently to make GBUAHN a house-hold name in Erie County.

As director of marketing, Sanchez facilitates and incorporates different marketing strategies to ensure that GBUAHN continues to expand its reach and success.


Dana IngebretsonDana Ingebretson
Registered Dietitian

Dana received her Bachelor of Science in Dietetics from Michigan State University in 2008. She moved to Buffalo in 2013 to attend the University at Buffalo’s Dietetic Internship program. After becoming credentialed as a registered dietitian, she received her Master of Science in Nutrition from UB in 2015. She has spent time working and volunteering in lower-opportunity communities to bring them improved nutrition and healthcare.


Erin M. NardozziErin M. Nardozzi
Compliance Officer

Erin joined GBUAHN in 2014. She received her Bachelor of Arts in Linguistics from the University at Buffalo and her Master of Public Administration from Hilbert College. She currently serves on the Board of Directors for the Buffalo Junior Chamber of Commerce. Recently, Erin became certified as a PCMH Content Expert by the National Committee for Quality Assurance.
